Musculoskeletal Rehabilitation in Singapore — Evidence-Based Movement, Exercise & Recovery

At The Pain Relief Clinic, we provide musculoskeletal rehabilitation that prioritises movement restoration, exercise-based recovery, and patient education.

Our approach is grounded in modern evidence: most musculoskeletal conditions improve best through progressive loading, structured exercise, and active rehabilitation—not passive treatment alone.

Our Core Philosophy: What Actually Works in Musculoskeletal Recovery

Across most spine and joint conditions, the evidence-based hierarchy of recovery is:

  1. Movement & Loading (Primary Driver of Recovery)
The most important factor in recovery is restoring normal movement and gradually increasing load tolerance.
  2. Exercise-Based Rehabilitation
Strengthening, mobility training, and progressive loading tailored to each condition.
  3. Patient Education
Understanding pain, recovery expectations, and avoiding fear of movement.
  4. Symptom Relief (Supportive, Not Primary)
Manual therapy, short-term medication, heat, or massage for comfort.
  5. Adjunct Therapies (Selective Use)
Shockwave therapy or electrotherapy in specific cases where appropriate.
  6. Medical Escalation (If Required)
Injections or surgery only when conservative rehabilitation is insufficient.

Conditions We Treat Through Rehabilitation-First Care

  • Back pain (mechanical and disc-related)
  • Neck pain (postural and nerve-related)
  • Slipped disc (lumbar and cervical)
  • Knee pain (mechanical overload and degeneration)
  • Knee arthritis (osteoarthritis)
  • Meniscus injuries
  • Shoulder pain (rotator cuff, impingement, tendon issues)
  • Frozen shoulder (adhesive capsulitis)

Our Rehabilitation Model (Physiotherapy-Led)

Our physiotherapy approach is structured, progressive, and tailored to each diagnosis:

  1. Assessment & Movement Analysis
We identify movement limitations, strength deficits, and load intolerance patterns.
  2. Diagnosis & Clinical Context
Medical input ensures we understand whether symptoms are mechanical, degenerative, or nerve-related.
  3. Exercise Prescription (Core Treatment)
Individualised strengthening, mobility, and control exercises form the foundation of recovery.
  4. Progressive Loading Program
Gradual increase in intensity to restore tissue capacity and resilience.
  5. Patient Education
Clear understanding of pain mechanisms, recovery timelines, and safe movement strategies.
  6. Supportive Therapies (If Needed)
Manual therapy or modalities used to support exercise—not replace it.
  7. Reassessment & Progression
Programs are continuously adjusted based on functional improvement.

Why Exercise-Based Rehabilitation Is the Most Important Treatment

Modern evidence strongly supports that the primary driver of recovery in musculoskeletal conditions is:

  • Progressive strengthening
  • Improved movement control
  • Load adaptation of tissues
  • Functional retraining

This is why passive treatments alone (massage, modalities, rest) often lead to short-term relief but not long-term recovery.

Role of Patient Education

We place strong emphasis on education because it directly improves outcomes:

  • Pain does not always mean damage
  • Imaging findings do not always correlate with symptoms
  • Movement is usually safe and necessary for recovery
  • Gradual progression is more effective than rest

Where Medical Imaging Fits In

Imaging is used selectively to support clinical decision-making:

  • X-ray — joint structure and alignment
  • Ultrasound — tendon and soft tissue assessment
  • MRI — disc, cartilage, or nerve-related conditions

Imaging supports diagnosis but does not replace rehabilitation as the primary treatment.

When We Escalate Care

Most conditions improve with rehabilitation. However, escalation is considered when:

  • Severe structural injury is confirmed
  • Progress stalls despite structured rehabilitation
  • Nerve symptoms worsen or persist

In such cases, we refer to orthopaedic or specialist care for injections or surgical evaluation when necessary.

How We Differ From Traditional Clinics

Compared to standard physiotherapy clinics

  • We integrate medical diagnosis with rehabilitation planning
  • We emphasise structured progression and education

Compared to pain relief-focused clinics

  • We do not rely primarily on passive symptom relief
  • We prioritise active recovery through exercise

Compared to orthopaedic-led care

  • We prioritise non-invasive rehabilitation first
  • We escalate only when clinically necessary

Our Care Model Summary

Assessment → Diagnosis → Exercise-Based Rehabilitation → Education → Supportive Therapy → Progression → Specialist Referral (if required)

Summary

The Pain Relief Clinic delivers structured, physiotherapy-led musculoskeletal rehabilitation focused on movement restoration, progressive exercise, and patient education.

We combine clinical diagnosis with evidence-based rehabilitation to help patients recover function, reduce pain, and prevent recurrence—without over-reliance on passive treatments or unnecessary procedures.

What To Expect When I Visit The Pain Relief Clinic

A typical visit will involve our doctor first understanding your medical history, concerns and previous experience with other pain treatments.

For patients who have consulted many people but have yet to receive a clear diagnosis, selecting an affordable imaging scan might be recommended to confirm the cause of your pain..

Some patients have already done scans with other doctors for their pain condition but are still not clearly told what they suffer from.

Dr Terence Tan is happy to offer you a second opinion and recommend how best to manage your condition.

We also see patients who already have a confirmed diagnosis from specialist pain doctors, but are "stuck” because treatment options offered are not practical or acceptable.

We can help by discussing options that you might have potentially never been told of.

A common experience is when a patient has already consulted a specialist doctor for pain management and is told to consider orthopaedic surgery which they find too aggressive.

Or they may have seen doctors for their pain and were prescribed painkillers with potential side effects which made them feel uncomfortable.

Many of our patients have also first tried complementary treatments or acupuncture with traditional Chinese pain doctors.

They look for a second opinion after finding any relief experienced from other treatments to be temporary or requiring repetitive treatments, which add up to time and cost.

Especially in such situations, we emphasize using non-invasive medical technology you likely have not been told about .

This can make a big difference to your results.
