Knee Arthritis Treatment Cost in Singapore: 2026 Guide

If you have knee osteoarthritis, one of the first questions you may have is:

“How much is this going to cost me?”

The answer depends greatly on what treatment you actually need.

Knee arthritis treatment in Singapore can range from relatively straightforward doctor assessment, medication and physiotherapy to injections, specialist procedures and, for some patients, knee replacement surgery.

But more expensive treatment is not automatically better treatment.

For many people with knee osteoarthritis, appropriate management starts with:

Doctor assessment → appropriate investigations → exercise and physiotherapy → weight/lifestyle management → medication where necessary → reassessment.

At The Pain Relief Clinic, we offer a structured $650 Knee Arthritis Rehab Programme, combining medical care and rehabilitation within the same clinic pathway.

For eligible patients in 2026:

Up to $500 MediSave may be claimable*

Cash payment from $150*

*Subject to individual MediSave/CDMP eligibility, remaining annual withdrawal limits, applicable co-payment requirements, sufficient MediSave balance and the claimability and clinical indication of individual services.

How Much Does Knee Arthritis Treatment Cost?

There is no single price for treating knee osteoarthritis because different patients require different levels of care.

Someone with relatively early knee osteoarthritis may primarily require:

  • Medical assessment
  • Education
  • Exercise
  • Physiotherapy
  • Lifestyle changes
  • Weight management where appropriate

Another patient may require:

  • X-rays
  • Medication
  • More extensive rehabilitation
  • Additional pain management

A smaller group may eventually require:

  • Knee injections
  • Interventional pain procedures
  • Orthopaedic specialist assessment
  • Knee replacement surgery

The important question is therefore not simply:

“What is the cheapest treatment?”

A better question is:

“What level of treatment do I actually need?”

What Are You Paying For When You Treat Knee Arthritis?

The overall cost can contain several separate components.

1. Doctor Consultation

A medical assessment can help determine whether the symptoms are actually caused by knee osteoarthritis and whether further investigations are necessary.

The doctor may assess:

  • Pain
  • Stiffness
  • Swelling
  • Walking ability
  • Knee movement
  • Functional limitations
  • Previous injuries
  • Previous treatment
  • Medication requirements
  • Other medical conditions
  • Whether physiotherapy is appropriate
  • Whether specialist referral is required

A medical consultation therefore serves a different role from simply purchasing a treatment session.

2. Imaging

Not everyone with knee pain needs expensive imaging.

For suspected knee osteoarthritis, an X-ray is often the relevant initial imaging investigation when imaging is clinically required.

MRI may be useful in selected situations, such as when the diagnosis remains uncertain or another knee problem is suspected.

One way to control unnecessary healthcare expenditure is therefore:

Use the appropriate investigation rather than automatically choosing the most expensive scan.

3. Physiotherapy and Rehabilitation

Exercise and rehabilitation are important parts of knee osteoarthritis management.

Physiotherapy may involve:

  • Strengthening
  • Improving knee movement
  • Improving hip and lower-limb strength
  • Functional exercise
  • Walking and movement advice
  • Exercise progression
  • Home exercise planning

A patient may require only a small number of supervised sessions together with a well-designed home programme.

Other patients may require more extensive rehabilitation.

The number of sessions should ideally be based on clinical need and progress rather than automatically purchasing a large package at the beginning.

4. Medication

Some patients require medication to help manage symptoms.

Medication costs vary according to:

  • Type of medication
  • Dosage
  • Duration
  • Medical history
  • Other conditions
  • Individual suitability

Medication can help control symptoms but generally should not be considered the entire long-term management strategy for knee osteoarthritis.

5. Weight and Lifestyle Management

For patients who are overweight, weight management may form an important part of knee osteoarthritis care.

But this creates a practical problem.

Someone may be told:

“Lose weight and exercise.”

Yet their knee hurts whenever they try to exercise.

A more useful strategy may therefore combine:

Knee rehabilitation + appropriate exercise + lifestyle/weight-management support.

This is why weight-management counselling can form part of The Pain Relief Clinic’s knee arthritis programme where appropriate.

6. Knee Injections

Some patients with persistent symptoms may consider injection treatment.

Options may include:

  • Corticosteroid injections
  • Viscosupplementation / hyaluronic acid injections

Different injections have different evidence, potential benefits, limitations, risks and costs.

They should therefore be considered according to the individual patient’s circumstances rather than assuming that an injection is automatically the next step for everyone with knee arthritis.

7. Advanced Pain Management

A smaller group of patients with persistent pain may be considered for additional pain-management strategies.

At The Pain Relief Clinic, visiting doctors can assess selected patients for options including interventional pain procedures such as pulsed radiofrequency, where clinically appropriate.

These are not routine first-line treatments for every person with knee osteoarthritis.

8. Knee Replacement Surgery

For some patients with advanced osteoarthritis and substantial persistent functional impairment, knee replacement may eventually become appropriate.

Surgery involves a very different level of expenditure from outpatient rehabilitation.

Singapore Ministry of Health bill information shows that costs can vary considerably according to whether treatment occurs in subsidised public care, unsubsidised public care or a private hospital.

This is another reason why patients should understand whether appropriate conservative treatment has been adequately attempted before progressing to surgery — while also recognising that surgery should not be unnecessarily delayed when it is genuinely indicated.

The Pain Relief Clinic $650 Knee Arthritis Rehab Programme

Rather than charging separately for each initial component, suitable patients can consider our:

$650 Knee Arthritis Rehab Programme

The programme includes:

2 Doctor Consultations

For medical assessment, treatment planning and progress review.

2 Physiotherapy Sessions

For knee rehabilitation, strengthening and exercise.

Individualised Rehabilitation Programme

To help patients continue appropriate exercise outside the clinic.

Lifestyle Counselling

Where relevant to long-term knee management.

Weight-Management Counselling

Where body weight may be contributing to knee loading and symptoms.

Knee X-Ray Where Clinically Indicated

When imaging is appropriate for assessment.

Medication Where Clinically Indicated

Based on individual medical assessment.

Why Publish a Fixed $650 Programme Price?

Healthcare pricing can sometimes be difficult for patients to understand.

A consultation may have one price.

An X-ray another.

Physiotherapy another.

Medication another.

Then follow-up consultation adds another cost.

The patient may not know the approximate overall cost until treatment is underway.

The purpose of the $650 Knee Arthritis Rehab Programme is to provide a clearer starting pathway.

Instead of simply asking:

“How much is one consultation?”

patients can understand what an initial structured programme of medical care and rehabilitation may cost.

Can MediSave Reduce the Cost?

Yes, for eligible patients.

Osteoarthritis is included under Singapore’s Chronic Disease Management Programme.

Qualifying outpatient osteoarthritis care may include eligible:

  • Medical consultations
  • Relevant investigations such as X-rays
  • Medication
  • Doctor-referred allied health services such as physiotherapy

For a patient without a complex chronic condition, the prevailing annual MediSave withdrawal limit in 2026 is:

Up to $500 per patient per year

subject to the prevailing CDMP rules.

Could the $650 Programme Cost Only $150 in Cash?

For an eligible patient who can utilise the full $500 MediSave amount towards qualifying components:

Programme price: $650

Potential MediSave: up to $500

Cash payment: from $150

However, it is important to understand the words “up to” and “from.”

Not every patient will necessarily pay exactly $150.

Actual cash payment depends on factors including:

  • CDMP eligibility
  • Available annual MediSave withdrawal limit
  • MediSave already used during the year
  • Available MediSave balance
  • Whether individual services qualify
  • Applicable co-payment requirements
  • The patient’s clinical requirements

Our clinic can explain the expected charges and applicable MediSave arrangements before treatment.

Is There a 15% Cash Co-Payment?

Under the prevailing CDMP rules, a 15% cash co-payment generally applies.

There is an important exception for qualifying patients receiving CDMP treatment at their enrolled Healthier SG clinic, where the usual 15% cash co-payment requirement can be waived under the prevailing rules.

This means the exact MediSave-versus-cash calculation can differ between patients.

For this reason, patients should not interpret “up to $500 MediSave” as a guarantee that MediSave will automatically pay $500 of every $650 programme.

Will the MediSave Limit Change in 2027?

Yes.

From 1 January 2027, the annual limit for patients without complex chronic conditions is scheduled to increase from:

$500 → $700

and the complex chronic-condition limit from:

$700 → $1,000.

Patients commencing treatment in 2027 should therefore check the prevailing limits at that time.

Private Physiotherapy vs Integrated Doctor + Physiotherapy Care

Cost should not be compared purely on a per-session basis.

A physiotherapy clinic may be appropriate for someone who already has a clear diagnosis and primarily requires rehabilitation.

But another patient may need:

Medical assessment

Investigation

Medication

Physiotherapy

Medical review.

In this situation, the relevant comparison is not necessarily:

“Which physiotherapy session is cheapest?”

It may be:

“What will my complete initial care pathway cost?”

At The Pain Relief Clinic, medical assessment and licensed physiotherapy are available within the same coordinated pathway.

GP vs Orthopaedic Specialist vs Physiotherapy Clinic: Which Is Cheaper?

Different providers perform different roles, so price alone does not determine which provider is appropriate.

General Medical Clinic

May be appropriate for medical assessment, medication and initial management.

Physiotherapy Clinic

May be appropriate where rehabilitation is the primary requirement.

Orthopaedic Specialist

Particularly important where significant structural pathology, complex disease or surgery is being considered.

Integrated Doctor + Physiotherapy Clinic

Can be useful for patients seeking coordinated medical assessment and conservative rehabilitation before considering more invasive treatment.

The Pain Relief Clinic primarily occupies this fourth category.

Is the Cheapest Knee Treatment Always the Best?

No.

But the most expensive treatment isn’t automatically the best either.

A better principle is:

Use the least intensive treatment that is appropriate and likely to help — then escalate according to response.

For example, someone who can improve with exercise, strengthening and weight management does not necessarily require an injection or procedure simply because one is available.

Conversely, repeatedly purchasing low-cost treatments that do not address the patient’s underlying functional problem may also represent poor value.

What If the $650 Programme Isn’t Enough?

The programme is intended as a structured starting pathway for suitable patients.

Some patients will require additional rehabilitation.

Others may need further medical treatment.

Depending on the diagnosis and response, additional options available through The Pain Relief Clinic and visiting doctors may include:

  • Further physiotherapy
  • Additional rehabilitation
  • Shockwave therapy in selected situations
  • TECAR-based treatment as an adjunct in selected patients
  • Corticosteroid injection
  • Viscosupplementation / hyaluronic acid injection
  • Interventional pain procedures such as pulsed radiofrequency in selected patients
  • Specialist assessment

Where surgery becomes appropriate, orthopaedic referral can be considered.

How Much Does Knee Replacement Cost in Singapore?

Knee replacement costs are substantially higher than outpatient conservative treatment.

Costs vary considerably according to:

  • Public versus private care
  • Subsidised versus unsubsidised status
  • Ward class
  • Surgeon fees
  • Hospital charges
  • Implant
  • Anaesthesia
  • Length of stay
  • Complexity
  • Insurance coverage

Patients considering surgery should refer to the latest Ministry of Health Hospital Bills and Fee Benchmarks and obtain an individual financial estimate from their healthcare provider.

The purpose of discussing these costs is not to imply that patients should avoid necessary surgery because it is expensive.

For someone who genuinely needs joint replacement, surgery may provide substantial improvement in pain and function.

The correct objective is appropriate treatment — not simply the lowest possible bill.

How Can I Keep Knee Arthritis Treatment Costs Sensible?

Several principles may help.

Get the diagnosis right.

Do not repeatedly treat “knee pain” without understanding what is causing it.

Use imaging appropriately.

Not everyone needs MRI.

Prioritise active rehabilitation.

Exercise and strengthening can continue outside the clinic.

Learn a home programme.

The goal should not necessarily be indefinite dependence on supervised sessions.

Address weight where relevant.

Reducing excessive knee loading can form part of long-term management.

Reassess treatment that isn’t working.

Do not simply repeat the same intervention indefinitely.

Escalate appropriately.

Additional treatments should be considered when clinically justified.

Frequently Asked Questions

How much does knee arthritis treatment cost in Singapore?

There is no single price because treatment ranges from medical assessment and physiotherapy to injections and surgery.

At The Pain Relief Clinic, suitable patients can start with a $650 Knee Arthritis Rehab Programme combining medical assessment and rehabilitation.

What does the $650 Knee Arthritis Rehab Programme include?

It includes two doctor consultations, two physiotherapy sessions, an individualised rehabilitation programme, lifestyle counselling, weight-management counselling where appropriate, X-ray where clinically indicated and medication where clinically indicated.

Can I use MediSave?

Eligible osteoarthritis treatment can be paid partly using MediSave under CDMP, subject to prevailing rules.

How much MediSave can I use in 2026?

For patients without complex chronic conditions, the prevailing annual limit is up to $500.

Different limits apply to complex chronic conditions.

Can I really pay only $150?

For patients able to use the full $500 MediSave amount toward qualifying components of the $650 programme, cash payment may start from $150.

However, this is not guaranteed for every patient because individual MediSave eligibility and co-payment requirements differ.

Is physiotherapy claimable from MediSave?

Doctor-referred physiotherapy for management of an eligible CDMP chronic condition can be MediSave-claimable, subject to prevailing requirements.

Should I see an orthopaedic specialist first?

Not necessarily.

The appropriate provider depends on the patient’s symptoms and condition.

Patients with suspected uncomplicated knee osteoarthritis who want conservative management may begin with appropriate medical assessment and rehabilitation.

Patients with severe functional impairment, significant structural problems, diagnostic uncertainty or possible need for surgery may require orthopaedic specialist assessment.

Is an MRI included?

No routine MRI is included in the $650 programme.

MRI is also not routinely necessary for every patient with straightforward knee osteoarthritis.

Where MRI is clinically indicated, the doctor can discuss the appropriate imaging options and costs separately.

Are knee injections included in the $650 programme?

No.

Injection treatment is not automatically required for knee osteoarthritis and would be considered separately if clinically appropriate.

Is shockwave or TECAR included?

These additional treatment modalities are not part of the basic $650 programme unless specifically stated.

They may be discussed separately for selected patients where clinically appropriate.

Start With the Appropriate Level of Care

What To Expect When I Visit The Pain Relief Clinic

A typical visit will involve our doctor first understanding your medical history, concerns and previous experience with other pain treatments.

For patients who have consulted many people but have yet to receive a clear diagnosis, selecting an affordable imaging scan might be recommended to confirm the cause of your pain..

Some patients have already done scans with other doctors for their pain condition but are still not clearly told what they suffer from.

Dr Terence Tan is happy to offer you a second opinion and recommend how best to manage your condition.

We also see patients who already have a confirmed diagnosis from specialist pain doctors, but are "stuck” because treatment options offered are not practical or acceptable.

We can help by discussing options that you might have potentially never been told of.

A common experience is when a patient has already consulted a specialist doctor for pain management and is told to consider orthopaedic surgery which they find too aggressive.

Or they may have seen doctors for their pain and were prescribed painkillers with potential side effects which made them feel uncomfortable.

Many of our patients have also first tried complementary treatments or acupuncture with traditional Chinese pain doctors.

They look for a second opinion after finding any relief experienced from other treatments to be temporary or requiring repetitive treatments, which add up to time and cost.

Especially in such situations, we emphasize using non-invasive medical technology you likely have not been told about .

This can make a big difference to your results.
