Knee Arthritis Treatment & Rehabilitation in Singapore
Doctor-Led Knee Osteoarthritis Care, Physiotherapy & Rehabilitation
Knee osteoarthritis can make everyday activities increasingly difficult — walking, climbing stairs, getting out of a chair, exercising, travelling or simply keeping up with work and family life.
But having knee arthritis does not automatically mean that you need surgery.
For many people, treatment begins with understanding the condition, improving strength and movement, managing pain, addressing contributing factors such as excess weight, and following a structured rehabilitation programme.
At The Pain Relief Clinic in Singapore, we provide a coordinated, doctor-led approach to knee osteoarthritis, with medical assessment and physiotherapy available within the same care pathway.
Our aim is simple:
Start with appropriate conservative treatment. Monitor your progress. Escalate treatment when necessary.

What Is Knee Osteoarthritis?
Osteoarthritis is a condition affecting the joint.
Within a healthy knee, cartilage provides a smooth surface that helps the joint move efficiently. With osteoarthritis, changes occur within the joint that can result in symptoms such as pain, stiffness, swelling and reduced function.
Common symptoms can include:
- Knee pain when walking or standing
- Difficulty climbing or descending stairs
- Pain when getting up from a chair
- Morning stiffness
- Reduced knee movement
- Swelling
- Grinding or cracking sensations
- Difficulty exercising
- Reduced walking distance
However, knee pain does not necessarily mean knee osteoarthritis.
Meniscal problems, ligament injuries, tendinopathy, gout, inflammatory arthritis and other conditions can sometimes produce similar symptoms.
This is one reason why appropriate assessment matters.
How Is Knee Osteoarthritis Diagnosed?
Assessment usually begins with your medical history and a physical examination.
Your doctor may assess factors including:
- Where your knee hurts
- How long you have had symptoms
- Activities that aggravate your pain
- Walking and mobility
- Knee movement
- Strength
- Swelling
- Previous injuries
- Previous treatments
- How knee pain affects work, exercise and everyday life
An X-ray may be appropriate when further assessment of the joint is required.
MRI is not automatically necessary for every person with knee osteoarthritis. It may be considered when there is diagnostic uncertainty or another knee problem is suspected.
The objective is not simply to find an abnormality on a scan.
It is to understand what is affecting you and what treatment is appropriate for your situation.
Knee Arthritis Treatment Should Be More Than Pain Relief
Pain relief can be important, particularly when knee pain is stopping someone from walking, exercising or participating in rehabilitation.
But successful osteoarthritis management should usually consider more than pain alone.
Singapore’s current clinical guidance emphasises an individualised approach that considers pain, function, quality of life, psychosocial factors and response to previous treatment.
Core management strategies include:
- Patient education
- Exercise
- Weight management where appropriate
Additional treatment can then be considered according to the patient’s symptoms and response.
At The Pain Relief Clinic, we use this principle to structure a progressive knee arthritis care pathway.
The Pain Relief Clinic Knee Arthritis Care Pathway
STEP 1 — ASSESS
The first step is understanding the knee rather than immediately choosing a treatment.
This may include:
Doctor consultation
Your symptoms, medical history, function and previous treatment can be reviewed.
Physical assessment
We assess how the knee problem is affecting movement and everyday function.
Imaging where appropriate
A knee X-ray may be arranged when clinically indicated.
Further imaging may be considered where necessary.
Medication review
Medication may be prescribed where clinically appropriate, taking into account the patient’s medical history and potential risks.
STEP 2 — REHABILITATE
Exercise and rehabilitation are important components of knee osteoarthritis management.
The Pain Relief Clinic has licensed physiotherapists in-house, allowing the doctor and physiotherapist to participate in the same overall treatment pathway.
Physiotherapy may focus on areas such as:
- Improving knee movement
- Improving lower-limb strength
- Quadriceps strengthening
- Hip and supporting muscle strengthening
- Improving functional movement
- Increasing confidence with physical activity
- Developing an appropriate home exercise programme
The objective is not simply to provide temporary pain relief.
We want to help patients improve what their knee allows them to do.
STEP 3 — ADDRESS WEIGHT AND LIFESTYLE WHERE APPROPRIATE
Body weight can substantially affect the mechanical load placed on the knees.
For patients who are overweight, weight management can therefore form part of an overall knee osteoarthritis programme.
However, there is an obvious problem:
“How am I supposed to lose weight through exercise when my knee hurts whenever I exercise?”
This is where coordinated care can be useful.
Instead of simply telling someone to “lose weight”, treatment can address both sides of the problem:
Improve the knee’s ability to move and exercise
while also
addressing nutrition, lifestyle and weight management.
Lifestyle and weight-management counselling can therefore be incorporated into the Knee Arthritis Rehab Programme when appropriate.
STEP 4 — REASSESS
Not everybody responds to the same treatment.
Progress should therefore be reviewed rather than automatically repeating treatment indefinitely.
Important questions include:
- Has pain improved?
- Can you walk further?
- Are stairs easier?
- Can you get out of a chair more comfortably?
- Has strength improved?
- Can you exercise more?
- Has your knee become less limiting at work or socially?
- Is your overall quality of life improving?
The next treatment decision can then be based on your response.
STEP 5 — WHAT IF BASIC PHYSIOTHERAPY IS NOT ENOUGH?
Physiotherapy and exercise remain important parts of osteoarthritis management, but some patients continue to experience significant symptoms.
Depending on the patient’s condition and clinical assessment, additional treatment options available through The Pain Relief Clinic and our visiting doctors may include:
- Additional rehabilitation strategies
- Shockwave therapy in selected situations
- TECAR-based therapy as an adjunct to rehabilitation in selected patients
- Corticosteroid injection where clinically appropriate
- Viscosupplementation / hyaluronic acid injection where appropriate
- Interventional pain procedures such as pulsed radiofrequency in selected patients
- Specialist assessment
These treatments are not automatically necessary for everyone with knee osteoarthritis.
Some have stronger evidence for particular indications than others, and benefits, limitations, risks and alternatives should be considered before deciding whether an additional treatment is appropriate.
STEP 6 — WHEN SHOULD SURGERY BE CONSIDERED?
The Pain Relief Clinic’s approach is non-surgical first when appropriate — not anti-surgery.
Some patients have advanced osteoarthritis with persistent pain and substantial loss of function despite appropriate conservative treatment.
In these circumstances, an orthopaedic assessment may be appropriate.
Current Singapore clinical guidance recommends discussing surgical or advanced pain-management referral when persistent knee osteoarthritis symptoms significantly affect function and quality of life.
Our role is therefore not to promise that surgery can always be avoided.
It is to help patients receive appropriate conservative management and escalate care when necessary.
Can MediSave Be Used for Knee Osteoarthritis Treatment?
Yes, eligible osteoarthritis treatment can be paid partly using MediSave under Singapore’s Chronic Disease Management Programme (CDMP) at participating clinics, subject to prevailing requirements and withdrawal limits.
Osteoarthritis is one of the conditions included under CDMP.
Depending on eligibility and clinical indication, claimable services can include:
- Medical consultations
- Relevant investigations, including X-rays
- Medication used to manage the chronic condition
- Doctor-referred allied health services such as physiotherapy
For non-complex chronic conditions, the prevailing MediSave withdrawal limit is currently up to $500 per patient per year.
Different rules apply to complex chronic conditions, and patients generally have an applicable cash co-payment unless treatment is received under qualifying Healthier SG arrangements.
Patients should therefore check their individual eligibility and remaining MediSave limit.
Why Having the Doctor and Physiotherapist in the Same Care Pathway Matters
Someone with knee arthritis may need more than one type of care.
A physiotherapist can play an important role in rehabilitation, strengthening and exercise.
A doctor can assess the medical condition, prescribe appropriate medication, order investigations where indicated and determine whether further medical or specialist treatment should be considered.
At The Pain Relief Clinic, patients can access medical assessment and physiotherapy within the same clinic pathway.
This makes it possible to coordinate:
Diagnosis → medical management → rehabilitation → reassessment → escalation when necessary.
For eligible CDMP patients, doctor-referred physiotherapy for the chronic condition may also form part of MediSave-claimable care, subject to prevailing requirements.
How Much Does Knee Arthritis Treatment Cost?
Treatment costs can vary considerably depending on whether someone requires only assessment and rehabilitation or additional imaging, procedures or specialist treatment.
We believe patients should be able to understand the likely cost of their initial treatment pathway.

Who May Consider the Knee Arthritis Rehab Programme?
The programme may be worth discussing with our doctor if you have:
- Diagnosed or suspected knee osteoarthritis
- Knee pain when walking
- Difficulty climbing stairs
- Knee stiffness
- Reduced mobility
- Difficulty exercising because of knee pain
- Recurring knee pain despite rest or medication
- Been told you have knee degeneration on an X-ray

- Been told that weight loss would help but find exercise difficult because of knee pain
It may also be appropriate for someone who wants to explore conservative management before considering more invasive treatment.
When Should You Seek Medical Attention More Urgently?
Not every painful knee is osteoarthritis.
Seek medical assessment promptly if you develop symptoms such as:
- A hot, markedly swollen knee
- Knee swelling accompanied by fever
- Severe pain after significant trauma
- Sudden inability to bear weight
- Rapidly worsening pain
- Significant joint deformity
- Unusually prolonged morning stiffness
These symptoms may require assessment for conditions other than routine osteoarthritis.
Frequently Asked Questions
Can knee osteoarthritis be treated without surgery?
Many people with knee osteoarthritis are initially managed without surgery. Exercise, weight management where appropriate, physiotherapy and selected medications are among the main conservative approaches. Additional treatments can be considered according to symptoms and response.
Surgery may become appropriate when symptoms remain severe and substantially affect function or quality of life despite suitable conservative management.
Should I see a doctor or physiotherapist for knee arthritis?
Both can play different roles.
A doctor can assess the diagnosis, review medical factors, prescribe appropriate medication, arrange investigations and determine whether additional medical or specialist treatment is necessary.
A physiotherapist focuses particularly on movement, strength, exercise and rehabilitation.
A coordinated programme allows these roles to complement one another.
Can I use MediSave for knee arthritis?
Osteoarthritis is included under Singapore’s CDMP. Eligible patients attending participating clinics can use MediSave for qualifying treatment subject to prevailing withdrawal limits and scheme requirements.
Can MediSave pay for knee arthritis physiotherapy?
Doctor-referred allied health services, including physiotherapy for an eligible CDMP chronic condition, can be MediSave-claimable subject to the prevailing scheme requirements.
Do I need an X-ray?
Not every episode of knee pain requires an X-ray.
Your doctor can determine whether an X-ray is appropriate based on your history, examination and clinical situation.
Do I need an MRI for knee arthritis?
Usually not as the first investigation for straightforward knee osteoarthritis.
MRI may become useful where the diagnosis is uncertain, symptoms do not fit the expected pattern or another structure or condition needs to be assessed.
Is exercise safe if my knee hurts?
Appropriate exercise is a mainstay of knee osteoarthritis management.
Some patients experience discomfort when beginning exercise, particularly after a period of inactivity. The programme should therefore be appropriate for the individual’s condition and progressed sensibly.
A physiotherapist can help determine suitable exercises and progression.
Can losing weight help knee arthritis?
For people who are overweight, weight management is an important component of knee osteoarthritis care because body weight affects the load placed through the knee.
Weight management and rehabilitation can be addressed together rather than treating them as unrelated problems.
What happens if physiotherapy does not work?
The first question is usually why symptoms remain.
Treatment may need to be adjusted, the diagnosis reconsidered or additional options discussed.
Depending on the clinical situation, these may include medications, selected adjunctive rehabilitation treatments, injections, advanced pain-management procedures or specialist orthopaedic assessment.
Are knee injections better than physiotherapy?
They serve different purposes.
Exercise and rehabilitation address strength, movement and function and remain central components of knee osteoarthritis management.
Certain injections may provide symptom relief in selected patients but should generally be considered as part of an overall management strategy rather than automatically replacing rehabilitation.
Is The Pain Relief Clinic an orthopaedic surgery clinic?
No.
The Pain Relief Clinic focuses primarily on doctor-led musculoskeletal assessment and non-surgical management, with in-house physiotherapy and rehabilitation.
Where specialist intervention is appropriate, treatment may involve visiting specialist doctors or referral for specialist orthopaedic assessment.
A More Complete Approach to Knee Arthritis
Living with knee arthritis should not simply mean repeatedly taking painkillers until the condition becomes severe enough for surgery.
Neither does every patient need injections, expensive investigations or advanced treatments.
The appropriate pathway depends on the individual.
At The Pain Relief Clinic, our approach is:
Assess the problem.
Start with appropriate evidence-based conservative management.
Combine medical care with rehabilitation.
Address weight and lifestyle where relevant.
Measure whether function is improving.
Escalate treatment when necessary.
For suitable patients, our $650 Knee Arthritis Rehab Programme provides a structured starting point combining doctor-led care and physiotherapy, with eligible treatment potentially claimable from MediSave under CDMP.

What To Expect When I Visit The Pain Relief Clinic
A typical visit will involve our doctor first understanding your medical history, concerns and previous experience with other pain treatments.
For patients who have consulted many people but have yet to receive a clear diagnosis, selecting an affordable imaging scan might be recommended to confirm the cause of your pain..
Some patients have already done scans with other doctors for their pain condition but are still not clearly told what they suffer from.
Dr Terence Tan is happy to offer you a second opinion and recommend how best to manage your condition.
We also see patients who already have a confirmed diagnosis from specialist pain doctors, but are "stuck” because treatment options offered are not practical or acceptable.
We can help by discussing options that you might have potentially never been told of.
A common experience is when a patient has already consulted a specialist doctor for pain management and is told to consider orthopaedic surgery which they find too aggressive.
Or they may have seen doctors for their pain and were prescribed painkillers with potential side effects which made them feel uncomfortable.
Many of our patients have also first tried complementary treatments or acupuncture with traditional Chinese pain doctors.
They look for a second opinion after finding any relief experienced from other treatments to be temporary or requiring repetitive treatments, which add up to time and cost.
Especially in such situations, we emphasize using non-invasive medical technology you likely have not been told about .
This can make a big difference to your results.
